Chronic Disease Management

If you have a chronic medical condition or require ongoing medical care, coordinated Chronic Disease Management (CDM) services may be of benefit to you.

Your general practitioner can develop a comprehensive plan to manage your chronic condition, which may include regular check-ups, monitoring of symptoms, medications, preventive care and ongoing management strategies. The practice nurses are often involved in coordinating and delivering your care. They are also available to answer any questions you may have and provide you with valuable health education and information.

CDM services can also include referral to allied health practitioners, including dietitians, exercise physiologists, psychologists and podiatrists for additional support and management. Your general practitioner, nurses and allied health practitioners will help you in achieving better health and monitor your condition(s).

Many people are unaware of the various health services available to them. CDM services are such an example and a great tool to help you. To be eligible for CDM services, you must have a chronic medical condition that has been present for at least six months or is likely to last six months. The condition must also require ongoing medical care and management of a GP and at least two other clinicians.

If you meet these eligibility requirements, you may be entitled to five Medicare-subsidised allied health services per calendar year, in addition to your regular GP appointments.

It is important to note that CDM services are only available for eligible patients and are not covered by all health insurance policies. Whilst your general practitioner may be able to determine whether you are eligible for CDM services it may be best to check with your provider before booking an appointment to avoid any unexpected fees.
