75+ Healthy Ageing Assessment
An annual Health Assessment is available to all patients over the age of 75 and is an in-depth look at all aspects of your health. It provides a great opportunity to review your overall health and your physical, psychological and social function and discuss with your GP any concerns you may have.
Each year, Healthcare on Collins invites our over 75-year-old patients to attend the practice to have their Healthy Ageing Assessment completed. It consists of a 45-minute appointment with our Practice Nurse, followed by a 15-minute appointment with your usual GP.
What happens during this Health Assessment?
The following will be completed by our Practice Nurse:
- Height, weight & blood pressure
- Smoking & Alcohol intake
- Sleep
- Immunisation status e.g. Flu, Covid, Shingles
- Physical function & mobility
- Home safety
- Social activities
- Balance assessment & Falls prevention
- Mood and memory tests
- Nutrition assessment
- Advanced care directives
Your assessment is then reviewed with your usual GP and recommendations are suggested to help you maintain your health, safety & independence at home.
Health Assessments are bulk billed (i.e. there is no out-of-pocket cost).
