After Hours & Emergency Care

Healthcare on Collins provides 24 hour care and home visits if required, by using a deputising service, National Home Doctor Service.

 

If you require after hours care or a home visit please call National Home Doctor Service on 13 7425 (13 SICK).

Any medical notes relating to a visit from National Home Doctor Service are forwarded to your doctor and included in your medical history.

The doctors at Healthcare on Collins do not routinely conduct home visits but in exceptional circumstances this may be arranged.

Transfer of Medical File

If you have decided to see a Practitioner at Healthcare on Collins, they may request your Medical File to have your medical history and provide the best care for you. Only the patient or their carer can make this request. You simply need to fill this form and return it completed to Healthcare on Collins who will liaise with your previous Medical Practice to obtain your file.

 

On the flip side, if you need to transfer your file from Healthcare on Collins to another clinic, please fill this form and give it to your new clinic.

Results of tests and investigations

Your Doctor might request for you to have some tests done. These tests are conducted by specialised pathology, imaging laboratories or other Specialist providers.

At the time of ordering the test, your doctor will advise you when to book a follow-up consultation to go through the results.

If you haven’t booked an appointment or haven’t heard from your doctor 7 days after a test, please contact Healthcare on Collins on 03 9650 4284.

You might be contacted earlier if your doctor needs to discuss your results sooner. Please ensure that you advise us of any change to your contact details.
